Job Summary
Stronger Roots is seeking an experienced and compassionate Licensed Clinical Social Worker (LCSW) to join our interdisciplinary clinical team.
This is a hands-on clinical position that combines direct client care with clinical leadership and oversight. We are looking for someone who is comfortable working directly with clients, conducting assessments, addressing complex behavioral-health needs, and collaborating with an interdisciplinary team to determine the best course of care.
The LCSW will conduct behavioral-health and psychosocial assessments, provide clinical interventions, participate in care planning and complex-case reviews, respond to behavioral-health concerns and crises, and provide clinical guidance to our teams.
The position will work closely with our Recuperative Care staff, nursing leadership, substance-use-disorder counselors, case managers, program leadership, and outside providers to support adults whose medical, mental-health, substance-use, housing, and psychosocial needs frequently overlap.
Key Responsibilities
- Conduct comprehensive behavioral-health and psychosocial assessments and identify mental-health, substance-use, and co-occurring conditions.
- Provide direct clinical services and behavioral-health interventions within the LCSW scope of practice.
- Develop, review, and provide clinical guidance regarding individualized care and treatment plans.
- Participate in Recuperative Care assessments, care planning, complex-case reviews, discharge planning, and interdisciplinary case conferences.
- Work directly with individuals experiencing co-occurring mental-health and substance-use disorders, serious mental illness, trauma, housing instability, chronic medical conditions, and other complex needs.
- Assess and respond to psychiatric symptoms, behavioral changes, changes in mental status, substance-use concerns, and other behavioral-health issues.
- Provide crisis assessment and intervention, safety planning, stabilization support, and referrals to higher levels of care when appropriate.
- Provide individual, group, supportive, and other evidence-based behavioral-health interventions when clinically appropriate.
- Collaborate closely with nursing leadership regarding individuals whose medical and behavioral-health needs intersect.
- Work collaboratively with SUD counselors to support individuals with co-occurring mental-health and substance-use disorders.
- Coordinate with hospitals, physicians, psychiatry, behavioral-health providers, SUD providers, and community partners to support continuity of care.
- Provide clinical consultation and guidance to case managers and program staff regarding behavioral health, trauma-informed care, de-escalation, professional boundaries, and complex client presentations.
- Support discharge and transition planning and connections to appropriate behavioral-health, substance-use, medical, housing, and community resources.
- Review clinical documentation and help ensure assessments, care plans, progress documentation, and clinical practices meet appropriate standards.
- Participate in quality assurance, incident review, clinical policy development, and continuous improvement of clinical services.
- Help strengthen clinical practices, workflows, and standards across the organization.
Required Qualifications
- Current, unrestricted California Licensed Clinical Social Worker (LCSW) license.
- Experience providing clinical services to adults with complex behavioral-health and psychosocial needs.
- Experience assessing and treating mental-health and co-occurring substance-use disorders.
- Strong skills in behavioral-health assessment, diagnosis, treatment planning, crisis intervention, and clinical documentation.
- Knowledge of DSM diagnostic criteria and evidence-based, trauma-informed approaches.
- Strong clinical judgment and the ability to recognize changes in condition and determine when additional intervention or a higher level of care is appropriate.
- Comfortable working directly with clients in community-based and residential environments, not solely in a traditional office or outpatient setting.
- Ability to collaborate effectively with nurses, SUD counselors, case managers, program staff, healthcare providers, and community partners.
Preferred Qualifications
- Experience with Recuperative Care/Medical Respite or programs serving medically and socially complex adults.
- LAADC, CADC-II, or another recognized California SUD/AOD credential in addition to LCSW licensure.
- Significant experience with co-occurring mental-health and substance-use disorders.
- Experience in residential SUD treatment, withdrawal management, residential behavioral health, or similar settings.
- Knowledge and practical experience with ASAM Criteria.
- Experience with DHCS-regulated behavioral-health or SUD programs.
- Experience working with individuals experiencing homelessness, justice-system involvement, serious mental illness, substance use, or chronic medical conditions.
- Clinical leadership, staff-development, or supervisory experience.
- Qualified to provide clinical supervision to eligible associates under California Board of Behavioral Sciences requirements.
